Liability Waiver
1. CoreWerx Waiver of Liability
I {name} release CoreWerx and all Instructors/ Teachers/ Facilitators from any responsibility and/or liability concerning the application, processing, and/or consequences of me participating in all activities, classes, courses, exercises, any and all services provided that I elected to voluntarily participate in these activities. I consent to have the abovedescribed services of my choice applied to me or on my behalf. I understand the risks associated with the above-mentioned activities, including but not limited to bumps, bruises, broken bones, soreness, stiffness, etc. and freely and voluntarily assume those risks that are inherent in these types of physical activity. As such, I release CoreWerx and the Instructor/ Facilitators, employees and its agents harmless against any and all liability, damage, and/or expenses arising out of or in connection with actions, claims, and/or damages resulting in personal injuries and disabilities (physical and/or psychological) or transmission of a communicable disease that I might incur as a result of the service provided today and I agree to voluntarily participate understanding these risks and their outcomes. I also affirm that I understand the risks of participating in the above described activities and that I have no physical or medical issues that would preclude me from to participating in these activities.
I {name} acknowledge and affirm that I have had a physical examination and/ or been given my physician's permission to participate, or that I have decided to participate in the activity without the approval of my physician and do hereby assume all responsibility for my participation and activities.
I {name} understand that all services and classes are non-transferable and non-refundable. I understand that classes and packages have an expiration date which is my responsibility to keep track of and will not be extended. I understand there is a 24 hour cancellation policy and I am responsible to reschedule my classes. If I do not reschedule my classes a cancellation and/ or rescheduling fee may be applied.
I {name} understand by signing this form that all photographs or videos taken at CoreWerx or any of their events outside the studio, may possibly be used for marketing purposes.
